| Additional Terms and Conditions
for Providers and Participants |
Health·Med·2 ("Copyright Holder") licenses the
use of an education-based Wellness Program
("Program") to independent health care practitioners
("Provider") who interpret, customize and deliver
the Program to individual participants
("Participants"). The Program consists of an
educational curriculum and various reports and data summaries
which are copyrighted by the Copyright Holder. The Program is
licensed by the Copyright Holder to the Provider, who in turn
customizes, interprets, and delivers the Program to
Participants based on their individual needs and
circumstances.
In some cases, Participants choose to use the Program
without the services of a Provider, and are therefore solely
and completely responsible for customization, interpretation,
and self-delivery of the Program. According to pre-agreed upon
Program Options, the Copyright Holder will send reports to
Participants and Providers which summarize data provided by
the Participant. These reports should not be construed as
recommendations by the Copyright Holder for either the
Participant or Provider. It is the Participant's sole
responsibility to contact a Provider of their choice if the
practice of medicine is needed in any way. The Copyright
Holder is neither a Provider nor practices medicine in any
way.
In no event will either Providers or Participants hold the
Copyright Holder responsible for the administration or results
of the Program. Furthermore, the Copyright Holder may require
Providers or Participants to agree to additional terms and
conditions. |
