Behavior Factors
  1. When you try to lose weight, do you find that you cannot keep it off long-term (more than 2 years)?

  4. Does your weight regularly fluctuate by 10 pounds or more?
  5. Do you often skip meals like breakfast, or eat late at night?
Risk Factors
  1. Do you have personal or family history of heart disease?
  2. Are you a male older than 45 years? 
  3. Are you a postmenopausal female?
  4. Do you smoke cigarettes?
  5. Do you have a sedentary lifestyle?
  6. Has your doctor ever told you that you have:
    - high blood pressure?
    - abnormal blood lipids (high LDL cholesterol, low HDL cholesterol, high triglycerides)?
    - diabetes?
